Intro
A tooth with poor prognosis, which would otherwise be indicated for extraction, can be salvaged by Hemisection.
The present case of hemisection followed the retreatment of a failed root canal treatment of #46, to salvage the distal half which had good prognosis.
Chief complaint - Pain in his lower right back tooth region, continuous in nature; history of Endodontic treatment 3 months back.
Diagnosis: Periapical abscess wrt 46 (perforation of the mesial root)
Treatment plan: Re- RCT wrt 46 followed by Hemisection to salvage distal root
Treatment done:
The endodontic re-treatment was initiated wrt 46, working length determined and BMP completed. The presence of sinus opening and lack of any discharge from the distal canal, implored the need for calcium hydroxide dressing, following which, the sinus opening resolved and obturation of distal canal was done.
This was followed by hemisection and and extraction of the mesial root(due to poor prognosis). Sutures placed.
On one week follow up, sutures were removed and patient was asymptomatic.
